Asking for proof that while off work for "FMLA" you were actually seen by the doctor?

I just began administering FMLA for our company. We have an employee that was approved for Intermittent FMLA. The individual took off work from 7-2-09 and returned to work on 7-13-09. His paperwork indicated that he would be off for work for gout, HTN and HLP (not all related). The doctor indicated that he would be off approximately 3-5 days for gout exacerbation. The doctor also indicated that based on his medical condition the employee could be off work 1x per 1-3 months. Can I ask for a doctor's note advising that the employee was seen by the doctor during the time frame of 07-02-09 thru 07-12-09? And for what?
